The first digit designates the 3rd party drug:

No third party drug = 0
metformin = 1
glipizide = 2
piaglitazone = 3
rosiglitazone = 4

The second digit designates vanadium:
No vanadium = 0
Vanadyl Sulfate = 1
bis(ethylmaltolato)oxovanadium (IV), or BEOV = 2

The third digit designates chromium:
No chromium = 0
chromium picolinate = 1
chromium nicotinate = 2
